The landscape of healthcare is undergoing profound transformation. While scholarly discussion has often centered on “rural medicine” as a means of addressing geographical inequities, Monthly Community Medicine has long embraced a broader and more integrative vision of “community medicine.” This discipline transcends mere geographical limitations, focusing instead on a holistic approach to supporting the daily lives and well-being of patients and community residents within their unique social contexts.
With this May issue, the journal enters a significant new phase. In a web based format published every two months, Monthly Community Medicine seeks to become a more agile forum for sharing the distinctive achievements and perspectives of Japanese community medicine with a wider international readership.
The present issue features three exemplary papers that embody the diverse facets of modern community medicine. In an original article, Sasai et al. highlight the role of community clinics in the control of infectious disease clusters1). In the review section, Kishaba offers an up-to-date overview of respiratory medicine, indicating the interface between primary care and specialist practice that is essential to improved patient outcomes2). Finally, Furukawa reports the use of brexpiprazole in a geriatric health services facility, illustrating how pharmacological intervention can restore the quality of life and nutritional status in patients with dementia3).
Looking ahead, we intend to incorporate new modes of scholarly communication, including article formats such as “Video Communication,” to deepen dialogue between researchers and practitioners. We warmly invite our readers to engage with this renewed journal. Together, we hope to shape the next chapter of community medicine.
月刊地域医学の新時代：日本の地域医療の実践を世界へつなごう

医療を取り巻く環境は大きな変化の時を迎えています．これまで地理的格差を埋める「へき地医療」に焦点を当てた学術論文はあれど『月刊地域医学（Monthly Community Medicine）』は，その先にある「地域医学」という，より広く深い視点を示してまいりました．単なる地理的な制約を超え，患者や住民がそれぞれの社会生活の中でいかに生きるかを包括的に支える医療です．本誌は今年度から，記念すべきリニューアルを遂げます．隔月発行のWeb版では本誌は日本の地域医療が培ってきた知見を，よりダイナミックに世界に向けて発信することを目指します．本号には，地域医学の多様な側面から３本の論文を掲載いたしました．
笹井平先生らによる原著は，地域診療所がPCR検査とゲノム解析を駆使し，感染症クラスター制御においていかに重要な役割を果たすかを述べています1)．喜舎場朝雄先生による総説では，地域でのプライマリ・ケアと専門的介入の橋渡しとなる呼吸器診療の最新知見をまとめ，アウトカム向上に資する連携のあり方を提示しています2)．古川智之先生は，介護老人保健施設におけるブレクスピプラゾールの使用経験を通じ，認知症患者のQOLや栄養状態への影響を報告しています3)．
本誌は「Video communication」論文など新しいコミュニケーション・ツールを積極的に取り入れ，実践者の対話をさらに豊かなものにしていきます．読者の皆様が，共にこの地域医学の新たな時代の幕開けを盛り上げてくださることを祈念します．
